Licensed Clinical Social Worker
To provide referral sources for individuals who are clients of the Sierra Vista Counseling Center. Develop and coordinate resources for all age specific client population groups. Review the assessment, planning and treatment of the social/emotional needs of clients and their families as those needs relate to and impact upon the client. To provide case management services. Work with psychosocial rehabilitation groups. Supervise Counseling Center clinical staff. - As SVH team members, we hold each other and ourselves accountable to our Standards of Excellence in everything we do. - Ability to supervise and conduct an assessment of the individual’s medical, social and functional limitations. - Develop and implement an individualized plan of care. - Mobilize the use of natural helping networks such as family members, church members and friends. - Develop increased opportunities for community access and involvement, i.e., assistance in locating housing, community livings skills, teaching, vocational, civil and recreational services. - Coordinate and monitor the delivery of services. - Ability to supervise and triage by level of urgency and need for services. - Recognize symptoms of acute and chronic mental illness. - Assess suicide risk and potential, and handle the situation. - Communicate and problem solve. - Establish rapport with persons with psychiatric disorders. - Assess and utilize community resources. - Provide culturally relevant services and/or demonstrate sensitivity toward ethnic minority groups. - Ability to supervise and determine if situation is life threatening. - Determine the dangerousness of the situation. - Attend to the legal status of the consumer. - Gain consent to notify family, when necessary. - Determine whether there is a known crisis plan and, if so, connect with the consumers’ caregiver. - Assure continuity of care and determine if seriousness of case requires continued attention. - Implement non-violent crisis intervention techniques. - Implement de-escalation and restraint techniques. - Anticipate and address client needs. - Able to adjust to changing job conditions. - Ability to handle stress.
